Flood determination content

Question: 
When completing a flood determination, is the borrower's name required on the form? Our audit department is listing it as a finding on their exam since the borrower’s name changed after the flood determination form was generated.
Answer: 

The borrower’s name is not required to be on the SFHDF. There isn’t even space provided for it. It could be this information is actually being required by your secondary market investor. We would recommend checking your contract.
