The notice has to indicate that the statement is available and provide an address and telephone number to which a request for a copy of the statement must be directed.
Although the first copy of the statement must be provided free of charge, that fact does not have to be included in the notice. I suggest that including that fact in the notice is a good idea, if you charge for more than one copy.
