If I have the following title, which exemption would I choose? If I have John Doe Co., Inc., DBA McDonald's, would it be a non-listed business (since John Doe is a non-listed business), or because it's a McDonald's franchise and McDonald's is a listed company, would it be a listed company?

I am completing a CTR for a representative payee and see that BOL says to complete two section A's with the representative payee on the first page and to describe his or her business as "Rep Payee". I am not sure if that means in box 5 DBA or box 13 occupation, profession or business.

We have a joint account for a sole proprietorship that is owned by a husband and wife as a DBA, but for tax purposes the business is filed under the wife's TIN. Should the DBA be listed in Part I, Section A, #5 for both the wife and husband on the CTR form or just the wife?

I need help with determining the correct way to file a particular customer's CTRs. We have a customer with two DBA accounts. For example: Jim Shoe Inc DBA Jim #1 and Jim Shoe Inc DBA Jim #2. Each DBA has it's own account number. It is my understanding that we should file a CTR based on the aggregate deposit total for the TIN in this case. If #1 has cash deposits of $5500 and #2 has cash deposits of $6500, then a CTR should be filed. Is this correct? Then, I'm not sure how to prepare the CTR. Should we put Jim Shoe, Inc DBA Jim #1 in Section A on the front and Jim Shoe, Inc DBA Jim #2 in Section A on the back?

I will be exempting a number of accounts for BSA. Some of these customers have more than one account under the same ID number but with different names, ie: Sandhar Corporation dba Marathon 4, and Sandhar Corporation dba Marathon 5. Do I need to do an Exemption form for each separate account, or can I do one form as long as the ID number is the same?
