I have an odd situation. A Teller completed a withdrawal for $15,000.00 from the wrong account and listed wrong the person. We completed a CTR but found out a few days later that all the information is incorrect.

We are not sure how to handle. Do we amend the CTR we filed with correct conductor and account information? Create a new CTR with correct? Try to recall?

I would amend the one you filed.

and make sure you put the $15,000 back into the account it was taken from in error, back credit the client with interest, and write them a letter of apology/explanation for when some government agent comes asking details...
