Does anyone have a checklist or questions that Tellers and CSRs are to ask before they just close out a customer's account?

We have one, that the customer must also sign if they are closing the account in person, that covers things like outstanding checks, automatic debits, sweep arrangements and the like. It also lists the steps the teller must complete as well.
