Govt Monitoring Form No Longer Required? HMDA

Our loan software provider has recently deleted the Government Monitoring Form from the list of required documents on loans subject to HMDA. Is this completed form no longer required on HMDA loans?.
Answer: 

There is no requirement for a specific form, but "a form" might be needed in certain situations such as commercial loans. Perhaps your loan software is for home mortgages using application forms with GMI built in. The bank would then be on its own for applications outside of that software.

There is a sample in the Getting It Right guide. The bank can create its own.
