Lost Passbook - Notary Required?

I have a client with a question regarding passbook savings accounts and I am unsure of the correct answer. The bank currently has a “Lost Passbook Affidavit” that must be signed by all account holders and notarized in order to issue a replacement book if a passbook is lost. There have been some recent changes to notary requirements in Ohio and the client is wondering if there are compliance implications to not having the Affidavit notarized. Can they change the affidavit and not require it to be notarized?

What you require to re-issue a passbook (I had no idea anyone even had any passbook accounts left) is a business decision. Talk to your legal counsel.
