Are financial institutions required to have a cardholder sign a dispute letter when filing a Reg E dispute on a debit card, or can a dispute be filed verbally and with no signature?
Answer: 

Actually the bank MUST accept an oral report. Refer to 1005.11.

The bank may require the consumer to give written confirmation of an error within 10 business days of an oral notice, but not doing so does not cancel the claim, but the bank won't be required to pay provisional credit in some circumstances if the written claim is not made.
