Refer to Section 205.6 and Section 205.11 of Reg E as well as your VISA/MC agreements. Reg E and the VISA/MC rules are very consumer oriented.
You can ask the customer to put the claim in writing, but if he/she doesn't, you must still resolve the dispute. You just don't have to provide provisional credit.
