For the Written List of Providers as required by RESPA, do the fees or range of fees have to be disclosed on this document? Also, does the document have to be exactly like the model form in Appendix D?

Answer:

The answer to your first question is no. The answer to your second question is also no, but only because Appendix D is the model form for the Affiliated Business Arrangement Disclosure. There is no model form in the regulation for the Written List of Providers. If you search the RESPA forum for the Written List of Providers you will find some samples.
