Check 21 Claim Form

"Bear Collector" has contributed two Claim Forms in Word format that can be used to assist customers in filing Expedited Recredit and other claims under Check 21 rules. The "Check 21 Claim Form" is designed to be completed by bank personnel. The "Substitute Check Claim Form" may be a bit more customer friendly, and includes additional tracking information on the second page. The Word documents are designed as fill-in forms. Unprotect the documents to customize them for your bank, then protect them again (for forms) before using them. Right click on any of the links to save the file to your computer.
