Does anyone know where I can find a sample form for new account expected activity? I have an old one that I feel asks way too many questions including actual salary information. Also, I've heard the CSR should ask the customer the questions and not really have them complete the form and sign it. Opinions?

There should be several forms in Bankers Tools related to new accounts and CIP.

A thoughtfully designed new account interview will serve several purposes. Service-oriented questions can help us cover many of our KYC issues. When we ask a commercial customer if they will be withdrawing large amounts of cash, we can do it from a cash service perspective (we want to make sure we always have sufficient cash on hand to service your cash needs). Inquiries related to wires can be supported by options to streamline the process through on-line connectivity or a call-in wire transfer agreement. The last thing we want is for our customers to feel as if they are undergoing an interrogation to open an account.
