Does anyone have a Heloc letter that they send to customers when the customer is requesting a payoff or after they have paid off a Heloc account?

I'm am looking for some type of letter to confirm with the customer if they want their account closed or not, requesting a signature to confirm closure.

If anyone does, would you be willing to share this information?
