I have a copy of a Handbook on Deposit and Fiduciary Accounts that references a Waiver of Heir form for distributing funds (less than $10,000) to a surviving spouse or next of kin. It's supposed to be Exhibit G, but it's not. Does anyone have a copy of this form from another version of this handbook? Thanks.

I have the same MA Bankers handbook with the same faulty table of contents. However, if you send me an email at jburnett@ccbt.com I have a Word document copy of the old exhibit G, which is the correct form. I'll be glad to send it back to you.
