We have loan which was originated with a six month term, and renewed in the same year. The CRA GIR guide only mentions when additional money is involved. This loan was renewed at the same amount. I know I can only report a loan once per year. Do I report the origination or the renewal? Or is it either one, at the institution's option?

Just report the first one. That is the normal practice - there would be no logic to reporting a renewal and ignoring the actual loan that was made in the first place.
