We currenlty have the basic privacy notice, no opt-out, nothing...we have a marketing company and they want to do a telephone survey with all of our customers....we will need to provide customer name and telephone number to them.

Q1. Do I need to change my privacy notice and;
Q2. Does this require me to do an opt out?

Any help would be greatly appreciated

If I am thinking correctly, you must change the wording in your disclosure to include that you disclose to the marketing company but this would be an exception for the opt out requirement, therefore no opt-out would be required.

Anonymous IS thinking correctly.

If you haven't done your annual notice yet, now's a great time. Anytime you make a change to the bank's privacy practices, you need to re-notify your customers. It makes sense to coordinate your changes with the annual notice requirement.
