Does anyone have an example of a disclosure as required by the E sign act for the customer to give consent to get the disclosure electronically?

If you send me your email address I can send you the e-sign disclosure we developed for use with our e-statement product. Also, if you search some of the "big" bank websites you will find examples of their e-sign disclosures as well.

The trick to ESIGN is not the language of the notice, but getting consent in a manner that demonstrates that the customer can receive and use a sample of the disclosure that's the subject of the consent.
