Do any of you use some form of standard template for Phase II customers when you do the annual review of CTR exemptions? If so, would you be willing to share? Thanks, Doug

I use our standard CDD Review Form for this review. I will happy to share. PM me your email address if you would like to look at. I also use a excel spreadsheet to show the number of transacations over $10,000 over the year

I also use a form--not the CDD review form but one that asks questions specific to an exemption. If you want to PM your email I would be happy to send it over to you.
