If your loan application form clearly states that marital status should only be completed if for joint or secured credit, but the applicant completes it anyway on an individual, unsecured application, is this a violation of Reg B?

Not in my opinion. You didn't ask for it but the applicant provided it anyway. I would either cross through it on the app and mark "did not request" or attach a memo stating what had happened.
