Regulation E allows either an oral OR written request for an access device. Is your bank requiring a written document signed by the customer for a debit card? For a replacement card (i.e. lost, stolen, etc.)?

I just came here researching this topic. For those that require written applications is there a specific reason (state reg.) or is it just "because that's how we've always done it"?

Since your debit card probably also involves a terms and conditions document some institutions want a signed application showing that the customer agrees to a contract. If you decide to accept a verbal check with bank counsel to ensure that you have a binding contract.
