Just wanted to confirm the correct 4789 forms that we are using are rev6/98. Is this the right revision date or is there a different date?
I need to make sure that we are using the correct date for BSA purposes.

Correct. You can always find the latest version of IRS forms at http://www.irs.gov/.

Until it gets revised, the way to distinguish the 6-98 version from previous forms is to look for a 4-digit year. This was a Y2K enhancement. I think our tellers finally understand that, but until recently we were still seeing submissions on 'old' forms.
