I have looked at the Model forms for error resolution and I cannot find any Model forms? Is there anything that we are required to disclose on these letters besides amounts and dates?

Notice that no error occurred in 1005.11(d) requires that you notify a consumer of their right to request copies of the documents you relied on to make your decision and also advise consumers that you will honor preauthorized checks and debits for five business days when you revoke the provisional credit.

There is no model language in the regulation.
