When credit life insurance is sold do you also provide an "Additional Insurance and third party payment addendum"? It basically states the bank retains a portion of the premium

There are requirements under TN Code Section 56-8-106(c) & (d) prior to the sale of insurance, but I am not sure that is one of the required disclosures.
