Wife has a DBA makes deposits into DBA accounts and personal account. Husband is on both accounts. The DBA has an EIN. Would I do one page one for the Wife, one page one for the DBA, and one page one for the husband? We are in Oklahoma.

First, you should never use an EIN for banking purposes when dealing with a sole proprietorship.

The sole proprietorship account belongs to the wife, even if the husband may be an authorized sign. Deposits into the joint personal account would be reported as on behalf of both parties.
