We had a customer select White for their race but also sign their name in the Principal Tribe field for Race. I know the reg tells us to report what they write in but this is very obviously a mistake. Should we document the error and report the correct information or should we report it as completed?

Doesn't necessarily sound like a mistake to me.....lots of people around here check both white and a Native American tribe. It's quite possible to be both......around here, not just possible, but common.

I just re-read your post....if you mean in the Principal Tribe field they wrote "John Smith".....i'd still report that, although yeah, that's a bit odd.

I have a GMI issue so I figured I would add onto this thread.

If an applicant selected both "Not Hispanic or Latino" AND "I do not wish to provide this information" on an online application. Would we report Not Hispanic or Not Provided? I would think Not Hispanic since the applicant still chose that ethnicity themselves, but I have never run into this issue before.

I would agree with reporting Not Hispanic. Just be sure to keep your documentation.
