Does anyone use a specific form when you are adding a name to an existing account?
If so, do you know where I can find a sample of the language that the form should contain.

We create a new/updated signature card with all signers and have a maintenance form signed by the original signer authorizing the addition. I would think you would need all signers on the same card.

We also complete a new signature including all signers and/or owners on the account and check the box "Replacement Card" on the signature card. It is also dated with the current date.

I agree with the others, but would also recommend disclosures to the "new" signer. If they were not on the account in the opening, they would not have received the necessary disclosures.

New disclosures (Reg. DD, Reg. E, Reg. CC, Privacy) are probably a good idea if the added person is new to your bank, but none of the regulations appears to include such a requirement. They all pretty much allow for one notice per account. But I'd recommend providing a bundle of disclosures if you expect the customer to know what's happening.

Of course, once our mandatory compliance deadline arrives for the Section 326 USA PATRIOT Act rules, you'll have to verify the ID of the added party and add their ID data to your collection of such things, along with whatever ID copy you have to keep.

We require a "new signature card" with all signers and give new disclosures. We do require he original owner of the account to be one making the request of adding the signer and be present to receive the new documents. This action covers any compliance updates since the account was opened. Since we all know that compliance is ever changing and our documents are always being revised for these changes. Thanks.
