When an application is approved but the applicant fails to respond to your notification or your commitment letter within the specified time. Who specifies the time? Does it automatically have a 30 day period similar to an Adverse Action? On a second note, if the loan is approved and you see the customer, in a non-business setting, and you ask the customer about their intentions with the loan. They tell you they changed their mind. Is that then a withdrawl and not approved but not accepted? Sorry for babbling along.

Once the credit decision is made, it would be an approved but not accepted loan request. You can specify any period of time you are comfortable with - 30 days - one week - what ever your bank policy. The regs don't determine how long you have to leave a commitment to lend open.

For HMDA reporting purposes, it would be classified as "approved but not accepted". How much time you give them to respond is up to you.

It appears that the applicant was notified of the approval within the 30 days, but has failed to act upon the approval by actually obtaining the loan.

You have your HMDA answer. The other question though is not if you approved it within the 30 days, but did you notify the applicant and tell them they were approved but needed to contact you by X date? As noted above, there is no prescribed period. 30 days is plenty long, 1 or 2 days would be of no use, as an example.
