I have a preapproval to purchase a house and under "Subject Property Address" on the application, it says "To be determined" and then has the city, state, and zip. It came from a mortgage company at first and was declined and then transferred to our bank and was also declined. I still have to report it though. What should I do for the address?

Thanks in advance!

From page A-5 of the GIR:

7. Requests for Preapproval. Notwithstanding paragraphs 1 through 6, if the application is a request for preapproval that is denied or that is approved but not accepted by the applicant, you may enter “NA” in all four columns.
