Upon receipt of an incomplete ACH authorization form, are we permitted to fill in missing information on the application and initial. This information would be for account number, social security #, and/or ABA number. This information would be obtained from the Customer profile for ss#, and the Customer's voided check or the ABA directory for account number and ABA.

I think we need a little more information here. I assume we are talking debits not credits. I think the answer is also dependent on what ACH application is being considered. Please be more specific and I'll try to help you.

Our deposit Customers may request to have their interest transfered to another account via ACH. Our ACH authorization form is required for a Customer to enact this debit. We sometimes receive the form back from Customers not completed. My question is - Is it OK for the financial institution to complete the form for the Customer?

I think that as long as you have the customer authorization to move the interest from that specific account and a copy of a voided check there is no harm in completing the rest of the information. If they fail to specify which account they want this to occur from, you may be in a grey area if they claim this is a problem.

Our form we usually fill out for the customer BEFORE they sign it. That way all the information is there and they have authorized that information. The employee receiving the form askes the cust to verify the information before signing it.
