Is anyone aware of any required information that a bank would want to put on it's substitute 1099 Int. form? I'm talking regulatory more than anything else. Obviously we have the TIN, Name and address, our 04#, but I'm wondering if we're required to put anything specific. Thanks.

The IRS Publishes that information each year in its general instructions document on forms 1098, 1099, etc. You can find this year's document in the back of IRS Publication 1220 or at this location.
