Is it required to have both the physical address of the originator as well as the beneficiary before sending out a wire transfer?

You only have to provide the address of the beneficiary if the originator provides it. Regarding the address of the originator - the answer is physical address or other acceptable address under the CIP regulations. See footnote 4 in this document:
