Do we have to give out a notice of delayed availability form to customers if we are placing an extended hold on a CD?

We give them for checking and savings but not sure if it is mandatory to give them out on CDs.

Reg CC only applies to checking accounts. So no. You also don't have to give them for savings accounts, but you may want to from a customer service stand point.

I've never heard of a bank delaying availability on a check used to open a CD. I'm not saying it's illegal - just never seen it before. You may want to check with your state department of banking as well.
