Exemption Codes on W-9

Question: 
Do we have to put an exemption code for a customer on the W-9?
Answer: 

Some of the newer signature cards with substitute W-9s ask for the exemption codes if the customer is exempt from back up withholding. If your signature card asks for this then yes you should put it. You can find them in the W-9 instructions at irs.gov. For example, a corporation has an exemption code “5”. These codes came in with FATCA in 2015.
