Where can I find a form letter that can be used to close a customer's account due to continuing activity from a SAR filing?

I have a question regarding the reissue of a cashier's check. We closed a CD for a customer and mailed the monies via cashier's check. The check is lost in the mail. Since the bank is the purchaser, can I reissue the check with the customer signing an affidavit of lost check? What is our liability?

When a loan is secured by a certificate of deposit held by the same organization should all names listed on the certificate sign the assignment if the C or D states "or" instead of "and"?

How long do we (Deposit Operations) need to keep customer statements returned for unknown address? Take into consideration that we would keep an Excel log of the ones that come back along with the reason for return.

We are looking to implement email notification of receipt of deposit instead of mailing paper back. I have seen comments on e-statements and E-sign and disclosure requirements but don't think any of that applies to receipts. Can you refer me to the regulation that states we are required to provide a receipt and the corresponding rules?

Many financial institutions that offer health savings accounts (HSA) simply request that the HSA application with initial deposit, and only a copy of an ID as verification be mailed, not presented in-person. With numerous HSA's being established out of state, no in-person verification is requested and seems to conflict with the PATRIOT ACT’s customer profile information requirement. How can our bank open HSA’s and not have the customer do this in-person, can this be done strictly via mail?
