Regarding a check stop payment request, after a customer has logged into our online banking site they have the ability to submit a stop payment request. Assuming they provide the required information to put a stop payment in effect - does the fact that the request was generated within the secured site allow us to forgo getting a signed stop payment order?

I have a question regarding Reg E. I have a customer disputing a Visa debit card transaction and she states that she was declined at a store, however a charge has been put through her account for $116.00. The dispute is entered, and came back "can not dispute as the pin and card were present". The customer did not lose her pin, or give the card to anyone else. Does the bank have to take the loss on this, and if so can the customer's debit card be revoked?
